Below are the health exam and prescription/over the counter medicine forms. All campers are required to have their health exam ready and with them on opening day, and you may download it here in PDF format. In addition the prescription/over the counter medicine forms are available at camp on opening day, but you can save time in check in by filling them in now.

ROPES RELEASE

All campers participating in the High Ropes program will need to fill one out on opening day. The same form will also be available at camp on opening day.
